VASER Lipo is a branded technology of high-frequency internal ultrasound equipment designed to emulsify (liquify) the targeted fat deposits immediately before they are treated with liposuction.
VASER technology utilizes titanium probes to deliver ultrasound heat energy directly to the surgical site. Ceramic skin ports and tumescent anesthesia protect surrounding structures.


​Potential Benefits

In areas of unusually dense or tough fat which are common along the back and in patients undergoing lipo for a second time, emulsficiation (liquefaction) reduces the physical strain on both the surgeon and the patient.

Liquefaction also loosens the non-fat connective tissue and reduces the likelihood that this tissue will produce unwanted skin creases or wrinkles.

Liquefaction of the targeted fat only (liposelection) leaves the deeper layers of fat in areas such as the upper thighs undisturbed and decreases the chance for unwanted indentations.

Newer Alternatives & Current Status

A decade ago, VASER was the only technique available for liquifying the fat layers. More recently, a faster and safer technique known as mechanical disruption has emerged and has replaced the need for VASER in most situations. However, VASER retains a useful role in areas of very dense, very fibrous tissue when adequate mechanical disruption cannot be achieved.

Our Experience

Our experience with VASER technology and techniques is unique. We worked with the VASER engineers to develop the safer modern protocols of tumescent anesthesia that are currently used today. We have also trained many surgeons in these techniques at the request of the VASER company as well as independently through more comprehensive lipo training courses.

Ultrasound-assisted Liposuction (UAL). Vaser Liposuction:
Intermittent or continuous bursts of ultrasonic energy (sound waves) are transmitted to the tip of the cannula to break up and liquefy fat cells which are then removed by suction. External ultrasound-assisted liposuction (E-UAL) uses external ultrasound waves to alter the fat cells in an effort to liquefy the fat before it is removed by suction.

Q. What type of liposuction is the best?

A.  All the liposuction techniques offer advantages and disadvantage; therefore, the use of specific liposuction equipment used alone or in combination with other liposuction devices, must be individualized based upon the patient needs, desires, technical feasibility and realistic expectations. At the Pelosi Medical Center all of the liposuction techniques are available. Drs. Pelosi use all the techniques of liposuction, tailoring them to the particular part of the body or patient. Be aware that many claims made by manufactures or physicians using a specific liposuction equipment system have been never been confirmed by rigorous clinical studies. They are only marketing strategies to lure potential customers. They use statements such as…”If your doctor doesn’t use our type of liposuction, you should look for a physician that uses it, because out system of liposuction is not really liposuction but something better.”

In general, the use of ultra-sound assisted liposuction including the Vaser® Liposelection require larger skin incisions and the need to suture them at the end of the surgery. They are also associated with more complications than the standard suction-assisted, power-assisted, and laser-assisted liposuction performed under local anesthesia. Such complications include burning and/or tingling, swelling, skin burns, and fluid collecting under the skin (seromas).

Q. How much liposuction is excessive (Staged liposuction, Megaliposuction)?

A. There is no universally agreed upon definition for excessive liposuction (Megaliposuction). Nonetheless, there is a limit to the amount of liposuction from which a healthy person can be expected to heal without significant risks. The patient's size and weight and the amount of fat aspirated are the main factors. As a general rule of thumb, Drs. Pelosi consider the aspiration of 4 liters of fat from a large patient the safe limit of conservative office liposuction, and a total aspirate volume of 6 liters should rarely be exceeded for one sitting liposuction session.

For patients who require megaliposuction exceeding the safe limit for one sitting liposuction procedure, Drs. Pelosi offer the alternative option of serial or staged liposuction. The megaliposuction is performed over the course of two or more sessions spread several days or weeks apart. Serial liposuction allows Drs. Pelosi to remove safely very large amounts of fat over a protracted period of time with no untoward complications.

Staged liposuction is a less invasive and safer alternative to bariatric surgery (gastrointestinal bypass procedures) for a significant number of obese patients in relatively good health. Liposuction will not treat the cause of the obesity, but it will result in a significant improvement of body contouring, and will remove large quantities of fat cells, while creating an adequate retraction of the excess skin.

Staged megaliposuction has shown that large quantities of fat can be extracted with minimal risk and that the large localized obesities can be definitely improved or eliminated while avoiding the significant morbidity associated with bariatric surgery. The operation is also beneficial to treat residual adiposities following bypass procedures.

Q. What is high definition liposuction (6-pack etching)?

A.  High definition liposuction is the three dimensional sculpting of the abdominal muscles, the chest muscles, the lower back and gluteus to enhance their appearance. The procedure is designed as an advanced scultping technique rather than standard liposuction and requires an experienced surgeon.

The procedure is indicated for males and females who have good muscle tone, exercise regularly, have good quality skin, and have been unable to achieve their desired muscle definition.

The procedure is performed under local tumescent anesthesia; the high definition liposuction goal is to remove the superficial and deep fat that covers the target muscles. Drs. Pelosi perform high definition liposuction using SmartLipo Laserlipolysis and Vaser Liposelection systems for the initial liquefaction of the fat around the muscles to be enhanced. The sculpturing is completed using fine cannulas specially designed by Drs. Pelosi for high definition sculpturing.

Out of Town Patients

Many lipo surgery patients at LVR-NJ travel considerable distances for our expertise in this procedure. We facilitate this process by providing the option of same-day consultation/same-day surgery. This is arranged in advance and by request at the time that you contact our office. Pre-operative bloodwork testing will need to be performed in advance through a local physician or lab near your home. Drs. Pelosi will review your situation to determine if this is an appropriate option. If you fly, or if your drive home is more than 2 hours, we recommend that you stay in the area overnight after your surgery so that Drs. Pelosi can evaluate you for proper healing on the day after surgery; afterwards, you may travel home.
